Vital Statistics

Birth Certificates:

 

We offer birth records from 1930 to present. Records on birth events that occurred prior to 1930 can be obtained from the Bureau of Vital Statistics.

 

Birth records under seal by reason of adoption, paternity determination or court order must be obtained from the Florida Bureau of Vital Statistics. For a record under seal write to:

Death Certificates:

 

Death certificates for deaths occurring in Martin County are available only at the Stuart office from 2007 to present. For death certificates prior to 2007, please contact the Bureau of Vital Statistics at the above address

 

You must be a family member or legal representative when requesting cause of death.

 

**A valid driver's license, passport, state or military identification card is required to process applications.**

A completed application should be submitted along with the appropriate fee. Make check or money order payable to the Martin County Health Department. Please do not send cash.

 

**A legible copy of a valid driver's license, passport, state or military identification card must accompany all requests.**
